Letter from a Captain

Dear Safety Delta,

My team recently completed the Diagnosis survey. To the surprise of the on-board leaders, including myself, the CSD Report showed very bad scores in leadership questions. As far as I know, the officers are doing a great job at promoting safety. In fact, we have never received any complaints from the ratings or the junior officers. Is the survey a performance evaluation for the top four? Are we the targets of the survey?

Thank you for sharing your concern.

We have received similar feedback from other vessels, and we understand that some of the survey results might be surprising. But to answer your question, no, the on-board leaders are not the targets of the Safety Delta Diagnosis survey.

The purpose of the survey questions

The Safety Delta Diagnosis survey aims to capture all crew’s opinions about the conditions on board. The anonymity of the survey can encourage the crew to open up about their honest perception of the safety culture, including the leaders’ ability to lead safety. This can help uncover safety-critical topics that are often difficult for the on-board crew to talk about in regular meetings. Hence, unsafe behaviours and practices are the true targets for development.

Not a performance evaluation

The Safety Delta Diagnosis survey is not a performance evaluation for any of the crew, not even for the officers. Poor survey results do not automatically translate to bad leadership. Low scores simply direct the vessel’s attention to safety topics that should be discussed in the Dialogue stage and key skills that can be developed during the Development stage. If the crew provide “correct” instead of honest answers in the survey, the results might not reflect the true on-board conditions. Consequently, potential safety issues will remain unresolved.
